Safe refine until +4.
After +3 → Fail = Gear breaks.
It can be obtained at: Monsters Drop, Boxes, Players, NPC Shop

Safer version.
Safe refine until +10.
Fail = Rollback to previous refine (e.g., +8 → fail → goes back to +7).
It can be obtained at: Battlegrounds, Raids, Void, Rift Fragments, Cash Shop, Vote Points Events, Players.

Used for refining after +10 (Lvl 1–4 weapons / Lvl 1 armors).

It never fails.
